1Microchipping Requirements

My pet has been microchipped with an ISO-compliant (15 digit, numerical) microchip if implanted on or after March 1st, 2019, OR with an ISO-compatible (10+ alphanumerical characters) microchip if implanted prior to this date.

I have proof of the date of microchip implant (proof of payment, microchip implantation certificate, or copy of pet's health record).

The microchip can be read (verified with veterinarian).

2Photograph

I have included a current, colour, full-body photo of my pet(s).

3Rabies Vaccination

My pet was vaccinated in accordance with the approved guidelines (see official documentation).

The rabies vaccination certificate includes make, serial number, date of expiration, and period of validity.

My pet was vaccinated against rabies AFTER the microchip was implanted.

💡 Note: The microchip and rabies vaccination can be done at the same appointment.

The microchip number has been entered correctly on the rabies vaccination certificate.

4Rabies Titre Test (FAVN Test)

The blood sample was taken at least 30 days after the rabies vaccination.

My pet has had a satisfactory blood test at an approved laboratory performed within the last 12 months (serum antibody level ≥ 0.5 IU/ml).

I have a copy of the blood test result showing the correct microchip number and the date the blood was drawn.

5Veterinary Health Certificate

A licensed/accredited veterinarian has correctly completed the veterinary health certificate.

The certificate has been endorsed by an official government veterinarian (if legally required by the exporting country).

I have received and will travel with the ORIGINAL health certificate.

When travelling, I will have my pet's vaccination record and blood test results, both showing the microchip number.

6Tick and Tapeworm Treatment

My pet was treated on the day of veterinary examination with approved treatments within 14 days prior to travel.

The products used to treat for ticks and tapeworms have been recorded on the veterinary health certificate.

7Travel Requirements

If travelling through a prohibited country, my pet's crate/container has been officially sealed.

My pet will travel in an IATA-approved container that conforms to IATA rules and regulations.
